You may apply by simply downloading the Application on the link below, completing it and mailing it to our main office. We also have a link for the Application Guidelines to explain everything to you in more detail.
You will be asked the following information:
Your place of residence
Your date of birth
Your ability to use other transportation
Your ability to walk on and off the van
Written proof of eligibility (proving your age, income or disability)
