You may apply by simply downloading the Application on the link below, completing it and mailing it in to our main office. We also have a link for the Application Guidelines to explain everything to you in more detail.
You will be asked the following information on the Application:
- Your place of residence
- Your date of birth
- Your ability to use other transportation
- Your ability to walk on and off the van
- Written proof of eligibility (proving your age, income or disability)
